New! Effective July 1, 2022, Blue Shield of California Medicare Supplement plans have a 12-month rate guarantee.
We offer a streamlined portfolio of Medicare Supplement products. They range from our basic Plan A, through our mid-range Plan N and Plan G, to our most extensive plans, Plan F Extra1 and Plan G Extra, and Plan G Inspire. 2 These plans help your clients get the benefits and price points they want, along with the extra services and support they need.
Our Medicare Supplement plans enhance Original Medicare and give your clients the freedom to choose any doctor who accepts Medicare. Our Medicare Supplement plans are designed to include affordable monthly rates, basic gym access at no additional cost through SilverSneakers®, and several savings opportunities 3 that can help keep monthly plan rates even more affordable for your clients.
Please note, Plan F Extra, Plan G Extra, and Plan G Inspire do not have a high deductible option.
1. Plan F Extra is available only to applicants who attained age 65 before January 1, 2020, or first become eligible for Medicare benefits due to disability before January 1, 2020.
2. Available in select counties in northern California. Please refer to the Summary of Benefits/Evidence of Coverage (EOC) for plan details and eligibility requirements.
3. Savings due to increased efficiencies from administering Medicare Supplement plans under this service pass on to the member.

For applicants from another carrier and existing Blue Shield Medicare Supplement members:
Eligible applicants MUST use the Application for Blue Shield of California Medicare Supplement plans (Form C12687) English (PDF, 504 KB), Spanish (PDF, 238) or the Blue Shield Medicare Supplement Online Enrollment Form. Please submit the appropriate application to ensure the coverage is effective on the desired effective date. The Blue Shield of California Medicare Supplement Plan Transfer Application will not be accepted. If received, it will be pended for the correct application.
Your clients will have to purchase a separate Medicare prescription drug plan for Part D coverage.

Who is eligible?
All California residents age 65 and older who are enrolled in Medicare Parts A and B at the time of application.
You may apply to enroll in Blue Shield's Medicare Supplement plans (A F Extra*, G, G Extra, or N) if:
- You are 65 or older
- You are a resident of the state of California
- You are enrolled in Medicare Parts A and B, Title 18, Public Law 89-97, at the time you apply
You may apply to enroll in Blue Shield's Medicare Supplement Plan G Inspire if you:
- You are 65 or older
- You are a resident of one of the following counties:

Individuals age 64 or younger who are entitled to Medicare because of a Social Security disability (if they qualify for guaranteed acceptance) unless they have end-stage renal disease may apply to enroll if:

- You qualify for guaranteed acceptance in a Blue Shield of California Medicare Supplement plan according to Blue Shield's guidelines

Medicare Advantage HMO and PPO Plans (MA-PD)
Blue Shield Medicare Advantage Plans cover the same basic benefits as Original Medicare plus Part D prescription drug coverage. Our plans include1:
- $0 monthly plan premium and $0 deductibles, depending on the plan and geographic area.
- Monthly Part B premium reduction, depending on the plan and geographic area.
- $0 copay for annual wellness visits
- $0 copay for primary care physician and specialist office visits, depending on the plan.
- $0 copay for preferred generic drugs when filled at our mail service pharmacy, CVS Caremark, or at these select retail pharmacies that offer preferred cost-sharing: Albertsons/Osco/Savon, Costco, CVS Pharmacy (including CVS Pharmacy at Target), and Safeway, Vons, Walmart, Ralphs1.
- Extended day supply fill options:
- 90-day supply available for maintenance medications on tiers 2 - 4
- 100-day supply available for maintenance medications on tier 1 - $0 copay for SilverSneakers® fitness and wellness programs, depending on the plan.
- Additional vision and hearing coverage and hearing aid benefits, depending on the plan.
- Optional Supplement Dental coverage with one of the state's largest dentist networks.
